You may order additional copies of the death record through the mortuary that handled the disposition, in person at our office (Death Certificates in Person), or by mail (Death Certificates by Mail). The application form Application for Certified Copy of Death Record and Sworn Statement and Acknowledgement are on line for your convenience. ALL information is required in order to process your request for certified copies. Our address is: HHSA Office of Vital Records and Statistics; 3851 Rosecrans St.; San Diego, CA 92110; Office Hours are: 9:00 am – 5:00 pm, Monday through Friday.
