The request must include the following information: • Title of material • How it will be used • Intended Audience • Number of copies to be made • If it will be used for educational purposes, a statement indicating that it will be distributed free of charge. Once received, your request will be reviewed and you will be notified within 4 weeks. A fee will be charged for commercial use.
