A Religious Exemption form (DHEC form 1126) can be obtained by requesting a form in person from any county public health department. Contact numbers for countypublic health departments can be found at http://www.scdhec.gov/health/countymap.htm. Forms cannot be e-mailed or mailed. Follow the instructions on the form. Don’t forget to have the document notarized.
