This is easy to do in Ascendis Caller ID version 1.0.3.0 or later. If you’re running an earlier version, download and run the latest installer here. Then, open the Options window (View menu -> Options) and put a checkmark next to “Start Caller ID when I log in to Windows”. Be sure to press “OK” to exit the Options window. Now Ascendis Caller ID should start automatically each time Windows starts.
