My son had reflux and we were told to put a blanket under his mattress to angle it. It wasn’t at such an angle that he would slip down but you could always roll up a blanket like a snake, form it into a “U” shape and put his rear end in the curved part with his legs hanging over the blanket. That way the blanket will keep him from slipping. The nurses did that to all the babies when both my kids were in NICU to keep them secure but also to keep the ones whose incubator mattresses were elevated from slipping.
