We know you may sometimes receive records from families that are more complete than information in the Registry. CIS forms are not considered medical records and currently are not considered a verified source of information.Therefore, it is not recommended to update the Registry from a CIS form. Your local Health Department may be willing to update the record if a family presents a shot record. You may wish to make a copy and forward it on to them.
