You need proof of who you are. The proof of residency and identity requirements for these are the same as those for a driver license. Documents would include: birth certificate, military ID, DD 214, permanent resident card, immigration visa, employment authorization card, military ID or U.S. passport. The process is the same as that for a driver license except that testing is not required for identification cards.
