(You might be able to ask Customer Service for an exception, but to take it a step further and complain to the FAA or USDOT because AA wouldn’t break their rules for you is excessive and won’t get you anywhere, in my opinion.) That having been said, though, most fare rules for nonrefundable tickets allow penalties and change fees required for reissuance of the ticket to be waived with a doctor’s note. Though it’s a longshot (since your dad is having the surgery, and not your grandmother), you might want to ask AA whether that is enough of a circumstance to allow the ticket to be changed without a change fee. If it’s not possible, she can simply cancel the ticket (saving the ticket number) and then using the value of the ticket (less the change fee) at a future date.
