How can I get a library card?
For adults and young adults (14-17 years): • Show a valid I.D. and proof of address, such as a driver’s license. • Fill out a brief application form. • Sign the card and accept responsibility for all items checked out.For children under 14: • A parent or legal guardian shows a photo I.D. • The adult fills out an application form and signs the child’s card. • The adult is responsible for all items checked out on the child’s card.You are responsible for all items checked out on your card. If you lend your card to someone else, you accept responsibility for the materials they check out on the card.

You may obtain a library card at the Belleville Public Library. To obtain a library card, fill out a library card form available from the librarian. Please bring two forms of ID proving your current address. One form MUST be a Driver’s License or State Issued Photo ID. Other acceptable forms that may be used as proof of address are a checkbook with current address, a utility bill, or utility deposit receipt. 2.
If you are a student, staff or faculty member of CSUDH, you already have a library card. The CSUDH ID card can also be used to check items out of the library. If you are not affiliated with CSUDH but still wish to check out books, you can pay a one-time fee of $30.00 to become a “Friend of the Library”. This will allow you to check out books from our stacks. Inquire at the circulation desk for more information.
Any person who resides within the Village of Brookfield will be issued a library card upon application without a fee. Applicants are required to provide proof of residence. Acceptable items of proof are a driver’s license or other photo ID, a checkbook with the current address, and mail such as a utility or phone bill. If the applicant has no proof of address, a postcard can be mailed to the residence and brought to the library as identification. The library card of an applicant under the age of 18 will be issued as soon as a parent or guardian has signed the statement of responsibility and proof of residence has been provided.
