First you will use the form 1040X I have linked below. All amended tax returns must be sent in, they are unable to electronically filed. Fill in the top of the form as shown. Be sure and indicate the calendar year at the very top, this will be 2008. In the Income and Deductions section, copy from your original tax return the information in both columns A and C as shown on the original return, down to line 15, This is where you will put the $7500 in column A and $8000 in column C. You will put $500 in 15 B. Then follow through the rest of this page as each line states. Sign and date also. On page 2 you will put an explanation in Part 2 of why you are making this change. You will also have to complete form 5405 to attach to your amended tax return before filing. I have linked it below. It should be just the same as you originally filed, except for the $8000 instead of $7500. It will take up to 12 weeks to receive a refund from an amended income tax return.
