To complete the on-line registration visit: http://www.costacruise.com/B2C/USA/before_board/declaration_new/PassengerDeclarationForm.htm Enter the Costa Cruise booking number (found in your invoice) and your last name Enter your Guest Information, including residence, proof of citizenship and emergency contact information for all passengers. Review and accept the Cruise Ticket Contract (CTC) Once the CTC has been accepted, print your Boarding form which you will need to bring with you (along with proper Proof of Citizenship and Cruise travel documents) the day of your departure. Failure to provide this information may result in a delay of document delivery, or a denial of boarding without refund.
