How can I change the address or phone numbers on the HOME page for the tournament?

You can sign in to the online sanction form at http://tournaments.usta.com/Tournaments/ click on Login at the bottom right hand corner and use the same login information that you use to sign into TDM. Click on your tournament name and go to Contacts tab where you can make those changes to any specific tournament. This will only affect those tournaments and not your actual facility membership profile.
