After the insurance period opens, new students must complete the health insurance application and life insurance beneficiary form. You can do this online through the HMSA website (www.hmsa.com/portal/student), or by submitting a hard copy application. Hard copy applications can be submitted by mail (see application for the HMSA mailing address) or by visiting the HMSA office at 818 Keeaumoku Street (processing time is 2-3 weeks). Those who enroll online must pay with a Mastercard or Visa and will be charged a $10 credit card processing fee in addition to the insurance premium. Those who enroll by paper application may pay by check or money order (made payable to HMSA), and will not be charged a $10 fee. Do NOT pay with cash.
