How can an employer disenroll a Preferred Care Gold member from his or her group contract?
Have the member complete and submit a Disenrollment Form or put his or her signed and dated disenrollment intent in writing to Preferred Care. Or, the Employer Group can notify Preferred Care through a billing adjustment worksheet and/or a letter that the member is no longer eligible for benefits through the group. Preferred Care will then move the Gold member to an Individual (Personal) Gold plan effective the first day of the month following the month in which the employer notification was received. Preferred Care will notify the member in writing that he or she has been moved to an Individual Gold plan and are responsible for paying the premium, or the member may choose to disenroll. If the member does not respond and does not pay premiums, he or she will be terminated in 90 days. Medicare controls the termination cycle.
