How can a person obtain a disabled parking permit?
An original Application for Disabled Parking Identification (Form 861) signed by an Alaska licensed physician must be submitted to the Division in order for an individual to obtain a disability parking permit. The application may be submitted in person or by mail. A permit may be issued for temporary or permanent disabilities. The disability must fall within the specific parameters of the definition of mobility impaired as defined on the application.
