How are usual, customary, and reasonable charges determined for out-of-network medical benefits?
Health Net, the Fund’s medical services provider, determines usual, customary, and reasonable charges for out-of-network medical services based on a fee schedule established by the Health Insurance Association of America within a particular geographical location. Health Net uses that figure, or the amount charged, whichever is less.
