How are teams placed in divisions?

There are a number of ways. The easiest is by drawing team numbers. For example if the league had six teams and decided to have two divisions of three teams each, then teams 1, 2 & 3 would make up one division and teams 4, 5, & 6 make up the other division. Or, if the team averages were known, the three highest average teams would make up one division and the three lower averaging teams would make up the other division.
