How are my clinical skills documented in the full provider education program?
After the successful completion of the online education program and online provider examination, a fully completed clinical skill verification form must be received, verifying required clinical skills by your hospital or local educator/clinical supervisor, before the provider card will be issued (forms will be sent to you with your ACLS or PALS Manual).
This clinical skills form will be sent with your provider manual. After the successful completion of the education curriculum online, the clinical skills form must emailed to info@acls-palsonline.org or faxed to 702-933-9059. This document may be completed by any supervisory clinical physician, RN, Paramedic or educator/clinical supervisor. This will complete your training records.
