Claims from the United States: In the U.S., your PPO provider generally files your claims for you on a HCFA 1500 form, either by mail or electronically. Non-PPO providers, if they file the claim for you, use this form also. The Federal Employees Health Benefits Program (FEHBP) requires providers to use this standardized method of billing for all claims, with the exception of foreign claims. If your non-PPO provider does not file the claim for you, simply send us the fully itemized bill, making sure to identify the insured member by name and Plan ID number. A fully itemized bill includes: Patient’s name and relationship to enrollee; Provider’s name and address; Diagnosis or symptoms; Date(s) of service; Type(s) of service; and Amount of charge.
