How are children tracked and who is involved?
If you have completed a common assessment and single agency support is required you will have agreed some actions for you or your service and or the child/family: delivery will involve you undertaking these actions, setting a date for review and monitoring progress. If however multi-agency support is necessary you will have identified actions for you and actions required by other agencies: delivery will involve you sharing your assessment with the agencies involved (subject to consent of the child/family); discussing and agreeing with the child/parent and agencies, establishing a multi agency management panel that will appoint the lead practitioner. Along with other agencies, you agree a multi-agency plan for delivering the actions, which can be recorded on the common assessment form. The lead practitioner will then monitor the overall progress of the plan.
