Course directors of approved programs (meetings) receive a “master” reporting form (VOICE Credit Reporting Form) which they copy and distribute to all participants. All participants are to complete the entire forms as directed. Technologist Section members and NMVTP participants return the top portions of the reporting forms to the course director who submits them, along with the complete list of participants, to the VOICE Coordinator in the Reston office for recording. Participants who are not members of the SNMTS retain the forms for their records. Participants are advised to learn the course director’s name should future contact become necessary. Participants of other types of activities who need their credit recorded, should follow the instructions that accompany each activity. For example, JNMT Continuing Education Articles require the participant to take the quiz and submit it to the SNMTS Continuing Education office for grading.
