Any allegation about a CARF-accredited program should be made in writing and sent to the CARF office. The CARF office, upon being informed by any source of a change in an organization’s operations in terms of the CARF standards or policies, may review and/or modify the accreditation status. Such a review may involve requesting from the organization an immediate and timely response to the allegations. The review may also involve the organization’s submission of documents and materials in support of ongoing conformance to the standards, soliciting information from relevant external organizations and/or individuals, and/or undertaking an announced or unannounced monitoring visit to the site at the discretion and expense of CARF. The survey team conducting the next site visit will also receive a copy of the allegation and any additional information relevant to the review.
