Schools new to 4Sight for the 2007-2008 School Year must register their schools on the 4Sight Member Center (on-line data management system for 4Sight) after submitting this form. To register on the 4Sight Member Center go to https://members.successforall.net/ and click on “Click here if you are registering to use the Member Center” and follow instructions.
