Benefits are available once every two years for a routine eye examination (eye refraction and dilation) and a complete pair of eyeglasses or contact lenses (in lieu of eyeglasses). Davis Vision has a network of licensed providers. Reimbursement for services depends on whether the member goes in or outside the network of providers. Participating/Network Provider Benefits: o No co-payment is required toward your eye examination, including dilation as professionally indicated. o No co-payment is required toward many spectacle lenses. o No co-payment is required toward a Premier frame from the “Tower Collection.” A complete benefit (frame and lenses) from the “Tower Collection” could have a comparable retail value up to $300.00. o A $25.00 or a $45.00 co-payment is required toward standard, soft, daily-wear, disposable, or planned replacement contact lenses. Your provider will give you specific information for the type of lenses you require. Out-of-network Benefits: Services for out-of-net
