Hi Tom – I’m not familiar with that form. Is it related to you wanting to take her name off the bonds or is it related to her saying she doesn’t know where the lost bonds are? If the former, consent of the beneficiary is required only with Series E and H bonds. I’m going to guess that these are EE or I bonds. If so, just send the form back without her signature. Include a letter respectfully requesting that the bonds be reissued because consent of the beneficiary isn’t required for the Series of bonds you have. (Let’s assume that that the clerk processing your request made a clerical error). If the latter (they want her to say she doesn’t know where the bonds are), send the unsigned form back with a letter explaining that she now lives outside of the US and obtaining her certified signature would be a hardship (which, believe me, it would be, even if she was willing to cooperate!). I’m less certain this will work, but it’s worth a try. Let me know how this turns out for you.
