If you don’t return to work for at least 3 months you will have to repay your maternity pay. Tax year doesn’t come into it I’m afraid.

No, once you leave the job, you are no longer their responsibility. You will need to sign on after 6 weeks and the Job Centre will pay the allowance. For clarification, check out the Revenue and Customs website.
