Health History and Physical Exam Form?
We should receive your Health History and Physical Exam Form at least three weeks prior to Camp. The physicians signed physical should have been performed within 24 months prior to Camp. A previous copy of such a physical or athletic card may be attached to our form. Let us know if you previously attended a Stevens Point Area YMCA resident camp or trip. If your physical on file from the previous year is within the 24 months we can use it for this year. A new updated Health History (first 3 pages) is still required every year.
