In the absence of a copy of the letter sent by the practice to HSE, (and for practices in business for a number of years and without any formal documents), the practice should phone HSE and obtain confirmation that they are registered, and make a telephone note of the name of the individual with which they have spoken, date etc. Ref: Standard 6.
