The hadware should not bury itself into the deck, this is a sign of over-tightening. This problem will also show up in drop through configurations due to metal on metal contact between the bolt head and the baseplate. One of the best ways to mitigate this stress point is to use a washer to spread the load. Slide a washer onto the bolt before you slide it through the baseplate on this type of configuration.
