Response: For Outpatient hospital SPUs and ASCs, do not use the Total Charge Revenue Code on the UB-92. This will cause your claim to reject. For Inpatient hospital claims, you should continue to place the 0001 Total Revenue Code at the top. Please note that the billing guides reference revenue codes in a four-digit format. Your claims will not reject if they are submitted in a three-digit format as PROMISe™ will add the leading zero to the Revenue Code for paper claims.
