No. Check the “No” box at the top of the cover sheet indicating that the case is not new. EAMS will already contain date of injury and body part information for existing cases. If the case is new, the “Yes” box must be checked at the top of the cover sheet and the date of injury and body parts must be listed on the cover sheet as well as the application.
