The formula used to establish a rate was 1.25 hours per person. However, to accommodate individual needs, some flexibility is allowed in the distribution of those hours. As long as the RN provides at minimum, a monthly evaluation of each consumer, the hours may be totaled and distributed based on needs within the home for Group Homes, Individualized Supported Living (ISL) and Residential Facilities; or totaled and distributed within the provider agency for multiple ISL’s if they wish. Allotted hours for Group Home and ISL settings should not be combined. Total hours should be provided each month and not be carried over into another month. Activity logs will be maintained by the RN to account for their time. This option to combine hours would require an activity log created per home or an activity log per agency for combined ISL’s. To simplify tracking hours, the nurse may elect to document the total time spent toward a particular activity on a given date rather than “time in / time out
