Physicians must be enrolled as a provider with Medicare. However, both PAR and non-PAR physicians may elect to participate in CAP. If non-participating, physicians accept assignment on a case-by-case basis to participate in CAP. By accepting assignment on a case-by-case basis, non-participating physicians agree to accept the allowable Medicare payment amount subject to the 5% reduction for non-participating physicians and not to bill the patient, other than for the deductible and other cost sharing.
