DOES THE PERORAL PNEUMOCOLON IMPROVE ACCURACY OF BARIUM RADIOGRAPHY IN THE TERMINAL ILEUM (TI)?
JK Marshall, R Cawdron, I Zealley, S Somers, EJ Irvine Departments of Radiology and Medicine, McMaster University, Hamilton, Ontario BACKGROUND: The small bowel meal with peroral pneumocolon (SBMP) is a modification of the standard small bowel follow-through wherein CO2, insufflated rectally, refluxes through the ileocecal valve and adds double-contrast (DC) TI images. Because the incremental yield of this approach is not well-established, a prospective study was initiated to compare the accuracies of SC and SC+DC imaging for the diagnosis of Crohn s ileitis. METHODS: 120 subjects with diarrhea and features of TI Crohn s disease underwent SBMP followed by ileocolonoscopy with TI biopsy. Two blinded radiologists (A & B) later issued diagnostic reports after reviewing (1) SC films then (2) combined SC+DC films. Films were later reviewed by two radiologists to achieve consensus. For each subject, a gold standard (GS) reference diagnosis was generated by a consensus panel of endoscopists (
