Does the OSMA have a model HIPAA authorization form or model consent to release information form?
The OSMA participated in the development of a Ohio specific model permission form for the use and disclosure of health information. The form, in two parts, is current as of December 2007 and includes all the elements required by Ohio and federal law. The form was developed by a group of Ohio health care attorneys working with the Health Policy Institute of Ohio. The OSMA encourages members to use this two part form. The “Permission to Use and Disclose Health Information for Treatment, Payment and Operations” should be used at the initial encounter with a patient to document the patient’s consent to release information for routine health care purposes. The “Permission to Release Health Information” should be used for all other purposes and complies with HIPAA’s authorization requirements.
