Currently, OPPT only offers the Toxic Substances Control Act Health and Safety Data Web form in CDX. One can, however, download a Adobe Acrobat version of the PMN by clicking on the link, below. More information about the downloadable PMN form is available by accessing this link: https://cdx.epa.gov/ssl/pmn/download.asp.
