Does the NACDD SHD Application Form need to include the names of all communities that are applying for funding from the other three national partners (NACCHO, YMCA, or NRPA)?
The only names of the communities you need to include in your NACDD SHD application are those communities that are applying for the NACDD funding. We do not need the names of the communities that are applying for NRPA, NACCHO, or the YMCA funding. This SHD application must link directly with those communities and or organizations that are applying for NACDD funding only.
