Does the Map of Medicine show integrated care pathways?
The Map of Medicine can be used as a clinical starting point for the creation of patient-centric integrated care pathways (ICPs) but, for a full definition of ICPs, NHS Wales staff should refer to the National Leadership and Innovation Agency for Healthcare’s online Guide to Good Practice. ICP and nursing documentation, referral and blood request forms and patient information can be added to local variants of the Map. The Map of Medicine should help save time and effort in that it supports the process flow work that is carried out at an early stage in full integrated care pathway development. It may also help inform clinical and case management of the triggers and routes in and out of ‘bundles’ of care (self-management or service provision) at the appropriate point in the clinical pathway.
