Yes. The Tribunal requires that the worker personally sign the Notice of Appeal form. It is not sufficient that the representative sign on behalf of the worker unless the worker provides a signed statement or form acknowledging that the representative has the authority to release the worker’s claim file to the employer and to any employers that appear to the Tribunal Counsel Office or to the Office of the VC Registrar as interested parties.
