No, the USAccess system does not issue temporary certificates, therefore name form or restrictions are not applicable. For termination or suspension of certificates due to unsuccessful background investigation, it is the responsibility of the Agency (i.e. Security Officer) to terminate or suspend the certificate. If the issue is provisional certificates, policies and procedures are already incorporated to accommodate this requirement. If the issue is forgotten passwords, policies and procedures to accommodate this requirement are the responsibility of the Agency.
