Does the final flash file need to be an EXE for the JSTART commands to work or will an SWF also work?

Yes it has to be a Projector file, it will not work on SWF files in a browswer due to security reasons, unless you are using the LOADMOVIE command to load additional SWF files from a projector file. See the FAQ above this one for more details.
