A provider may not charge a fee for writing a standard report that is generated in the normal course of treatment (e.g., office visit documentation). If the provider writes a special report that is unusual or outside the standard reporting forms, then an additional fee may be charged. The fee schedule does not set a fee for the usual code that identifies a special medical report, CPT 99080, nor does it show the default of POC 76. Whenever the fee schedule does not cover a procedure, the usual and customary rate would apply. The fee schedule does not cover fees for copying medical reports.
