Does the direct service staff or person who directly observed/had awareness of the incident need to write the CIR?
A. Anyone who has directly observed or become aware of a critical incident can fill out the form. Direct service staff may confer with administrators for help in filling out the CIR. Administrators/supervisors may also fill out the form. The 72 hour reporting period allows for follow up time for providers to identify the incident’s contributing factors, and form/implement a supportive response. The CIR asks for information about the person and incident, and for provider response.
