Does project slippage (exceeding planned finish date) require a Project Change Request? How is it dealt with?
Planned finish dates are static information, which is part of the project baseline; they do not change. Actual finish dates are recorded (and dealt with) by the Project Manager, and do not require a Change Request in and of themselves. However, if the root cause that is causing the slippage is a scope change, then that would require a Change Request, and if approved, the Project Plan and Schedule would be adjusted accordingly.
