There is an issue with Polyarchy running on Windows 2003 SP1. Here is a workaround: This is in the event log: Event Type: Error Event Source: DCOM Event Category: None Event ID: 10016 Date: 4/27/2005 Time: 1:33:56 PM User: NT AUTHORITY\NETWORK SERVICE Computer: COMPUTERNAME Description: The machine-default permission settings do not grant Local Activation permission for the COM Server application with CLSID {C9E717CA-B85F-41C5-8102-29A672A583D9} to the user NT AUTHORITY\NETWORK SERVICE SID (S-1-5-20). This security permission can be modified using the Component Services administrative tool. Looks like this fixes the issue.
