This was selected as Best Answer Phaniraj, Pharmacovigilance is a very broad concept that includes the detection, study and prevention of adverse drug reactions. It’s usually targeted towards registered drug products that are on the market (i.e. postmarketing drug surveillance), but it may also start earlier during the clinical study phases of a new compound in order to be able to evaluate as soon as possible the risk of medical treatment when given to larger groups of patients. So, to answer your question, it’s certainly not limited to phase IV.
