Yes, BUT ONLY IF YOU ARE LICENSED FIVE OR MORE YEARS IN ANOTHER STATE. 1) Applicants who are licensed 5 or more years and listed in the National Register of Health Service Providers can, in addition to having the National Register submit an official letter of Certification to the Board office, omit the Supervisor-Collaborator Forms from their application, as well as the Internship Director Form. An official score report from ASPPB is still required, as are ALL other parts of the application. However, such applicants will not be denied if their coursework fails to conform to current licensure requirements. 2) Applicants licensed 5 or more years who hold the CPQ must submit all parts of the application, in addition to having ASPPB send an official score report and an official certification of the CPQ.
