Yes, Online Applications require supporting documentation based on the type of request. Typically, required documents include, (but are not limited to): • Program Line-item Budget with budget narrative • Program Cash Flow • Current year balance sheet and/or P&L for the organization • Most recent audited financial documents for the organization • IRS determination letter • Board roster • Any other documents necessary to fully review your request Supporting documentation may be in the form of reports, spreadsheets or documents in Microsoft® Office, Quicken® or PDF formats that are part of your organization’s normal administration—OR—you may use one of the template forms at “Supplemental information” on our “Apply Online” page.
