Does Colorado have a state-specific hazardous waste manifest form? Does the State supply copies of manifest forms?

US EPA revised the Hazardous Waste Manifest Forms used to track hazardous waste from a generator’s site to the site of its disposition. These new forms must be used after September 5, 2006. Waste handlers can obtain new forms from any source that has registered with US EPA to print and distribute the form. Colorado does not supply copies of manifest forms.
