Does anyone know where to get the Farnsworth D-15 color vision test done?

I’m working on the last part of the medical hold which is getting the Farnsworth D-15 color vision test for the Border Patrol. So far I’ve gone down a list of doctors from the phone book in my area (Colorado Springs, CO) but haven’t had any luck finding one. I’m hoping that someone here had to have one done and can share where they went. Thanks!
